Prescription for a Healthy Oakland
Introduction
You are being asked to be in a research study that is being done by Oakland County Health Division and Oakland University (Principal Investigator is Dr. Laurel Stevenson, Associate Professor, Interdisciplinary Health Sciences).
Your decision to participate in this study is voluntary. You can choose to stop your participation at any time or skip any part of the study if you are not comfortable. Your decision will not affect your present or future relationship with Oakland University, the researcher, the Department of Interdisciplinary Health Sciences, or Oakland County Health Division.
What is the purpose of this study?
The purpose of this research is to understand how participating in Prescription for a Healthy Oakland impacts food security, dietary intake, chronic disease, and other social determinants of health.
Who can participate in this study?
You are being asked to participate in the study because you are over the age of 18, currently receiving or eligible for SNAP benefits, reside in Oakland County, and able to access the internet and shop at participating vendor locations. The pre- and post-program surveys will be offered in English and Spanish.
What do I have to do?
You will be asked to complete a pre-program survey and a post-program survey about food security, dietary intake, chronic disease, and other social determinants of health. The surveys will be done online and will take approximately 10-15 minutes each to complete. The pre- and post-surveys will be 6 months apart based on the duration of the program.
If you agree to take part in this research study, you will be asked to complete the survey online at a time and place of your choosing. After completing the program, you will be redirected to a SECOND, SEPARATE survey. This survey will ask for your name. This information will not be linked to your data in any way. Please note that each month of the program, you will watch a health education video ranging from 10-20 minutes each. The videos are sent to you by the program regardless of your participation in this research.
Are there any risks to me?
For this study, the potential risks are minimal. There may be a risk that someone who is not part of this research may accidentally see your personal information. We will try to make sure that this does not happen by keeping your research records as confidential as possible. All data will be stored via Qualtrics and Google Excel on a shared drive through the Oakland University system. The shared drive will be password-protected and shared with only those team members involved in data collection and management of the program.
When the results of this research are published or presented at conferences, no information will be included that personally identifies you.
Are there any benefits to me?
There may be no direct benefits to you, however, the results of this study may benefit others in the future.
Will I receive anything for participating?
If you choose to participate in this study, you will receive two small non-monetary incentives upon completion of the pre-program survey and upon completion of the post-program survey at the end of the study.
What if I want to stop participating in this study?
If you want to stop participating in the study, close your browser before completing and submitting the survey. After submitting the survey, it is not possible for you to withdraw your information. If you wish to stop your participation after submitting the survey, please contact the researchers and they will delete your information.

The financial sponsor of this study is the USDA Gus Schumacher Nutrition Incentive Program. Key program personnel and Oakland University researchers have no financial conflicts of interest.
