After completing the required parts of Session 5, you will have officially completed the Prescription for a Healthy Oakland produce prescription program!! This is an accomplishment and the whole Prescription for Health team is grateful for your commitment.
In addition to the video and quiz for this month you will also be asked to complete a post program survey. Both the Session 5 Quiz and Post-Program survey need to be completed before vouchers will be mailed.
